If you do not require a tax receipt, you can make your payment directly to the order of the Centre de Santé de la Vallée Massawippi (CSVM).
However, if you do require a tax receipt, then please make your cheque payable to the order of Fondation de Santé de la Vallée Massawippi (FSVM) and a charitable tax receipt will be issued by the FSVM.
For donations payable to the order of FSVM, please use one of the following two options to ensure the funds are donated to the CSVM Nurse Practitioner Initiative.
Option 1: Indicate on your cheque that your donation is designated to the Centre de Santé de la Vallée Massawippi (CSVM) Nurse Practitioner Initiative.
Option 2: Indicate on your cheque that your donation is designated to the Dian Cohen CSVM Fund, which has been established to support CSVM and its ongoing healthcare mission.
